Equipment Finance Quote Form: Fields That Matter
Build an equipment-finance quote form around company, equipment type, estimated amount and timing without collecting sensitive credit or underwriting documents too early.
Overview
A good equipment-finance quote form should help the finance team answer:
That is enough for the public website.
The full credit file belongs later.
- What equipment is being purchased?
- What does it cost?
- Is the buyer a real operating business?
- Is a vendor/dealer already identified?
- When is the purchase expected?
- Which specialist should follow up?
Field 1: Company and contact
Collect:
- company name;
- contact name;
- work email;
- phone;
- state/region.
Field 2: Equipment type
Use the provider's real categories, for example:
Current equipment-finance sites frequently make equipment type a first-step field.
- construction;
- truck/trailer;
- medical/dental;
- manufacturing;
- agriculture;
- technology;
- office/printing;
- material handling;
- other.
Field 3: Estimated equipment cost
Use either:
Examples:
Use ranges that match the provider's actual programs.
- a numeric amount; or
- broad ranges.
- under $25k;
- $25k–$100k;
- $100k–$250k;
- $250k–$500k;
- $500k+;
- not sure yet.
Field 4: New or used
Useful options:
- new;
- used;
- mixed;
- not sure.
Field 5: Purchase source
Ask at a high level:
Only include sources the finance provider actually supports.
- dealer;
- vendor/manufacturer;
- private seller;
- auction;
- not selected yet.
Field 6: Time in business
Current quote builders sometimes use broad time-in-business bands.
Use a simple range, such as:
Do not use the public form to issue a credit decision.
- startup / under 1 year;
- 1–2 years;
- 2–5 years;
- 5+ years.
Field 7: Industry
This can help route equipment expertise.
Keep the list short enough to use.
Field 8: Purchase timing
Ask:
- immediately;
- within 30 days;
- 1–3 months;
- 3–6 months;
- budgeting/research.
Field 9: Vendor/dealer identified
Use:
This last option can route partner leads cleanly.
- yes;
- no;
- still comparing;
- I am the vendor/dealer.
Field 10: Finance goal
Useful choices can include:
- purchase equipment;
- lease equipment;
- refinance/leaseback where offered;
- replace existing equipment;
- expand fleet/capacity;
- discuss options.
Field 11: Broad credit context only if the provider truly uses it
Some live quote builders ask a self-reported credit estimate.
If used, make the purpose and privacy handling clear.
Do not request:
inside an ordinary marketing form.
- Social Security number;
- tax ID;
- date of birth;
- bank credentials;
- hard-credit authorization;
Field 12: Notes
Prompt:
Tell us what equipment you want, where you plan to buy it and anything about the timing or transaction we should know before we call.
What the public quote form should not collect
Avoid requesting:
Move those into the secure finance application.
- tax returns;
- bank statements;
- government ID;
- guarantor data;
- ownership verification documents;
- full financial statements;
- credit-card or bank-account details;
- signed credit authorization.
Separate quote from secure application
Use two stages:
Public quote
Fast, low-friction, commercial context.
Secure application
Formal borrower, ownership, credit and document workflow.
The public quote should not pretend to replace the second stage.
Confirmation message
Use:
Thanks. We received your equipment-finance quote request. A finance specialist will review the equipment, estimated amount and timing and contact you about the next step.
Avoid:
You are approved.
or:
Your financing is guaranteed.

Questions businesses ask
Should equipment cost be required?
Usually yes, at least as a range. It helps route the request to the right program.
Should credit score be required?
Not in every public quote flow. If the provider uses a self-reported range, make clear that it is not a credit decision and move formal credit authorization into the secure application.
Should the quote form accept an invoice?
A vendor quote or equipment invoice can be useful, but use the provider's approved secure document workflow rather than assuming ordinary website storage is appropriate.
Does OneStudio underwrite the deal?
No. OneStudio should remain the public website, quote/enquiry and ordinary CRM layer.
